Providers: Make A Referral

Referrals from psychiatrists, neurologists primary care physicians, psychologists  and other mental health providers are welcome. 

Dr. Sinha works collaboratively with other health care providers when coordinated patient care is required.

Do remind your patient to make the initial contact with Dr. Sinha's practice.

For new patients, on your first visit, you will be asked if you wish for Dr. Sinha to communicate with the provider who has referred you for services. Please be sure to bring the contact information (name, address, telephone and fax numbers) of your primary care physician, psychiatrist or neurologist if a referral request was not submitted prior to your visit.
